Terms & Conditions for User Access to UCI Health MyChart Patient Web Portal

UCI Health is pleased to offer you the ability to view parts of your medical record and interact with your physician’s office online by using UCI Health MyChart. This web site and related services are subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of UCI Health MyChart will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit UCI Health MyChart.

Your privacy is important to us. UCI Health will use your confidential medical information in order to provide you healthcare services. We take many precautions to maintain your confidential medical information in strict confidence and will not deliberately disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law. Please review our Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within UCI Health MyChart that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.

A. Use of MyChart

NEVER send any messages requiring urgent attention using UCI Health MyChart. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume & availability of staff. Therefore, for all urgent medical matters, contact your health care provider's office immediately, go to an emergency room, or call 911.

B. Eligibility

You must be an active patient of a healthcare provider within UCI Health who has elected to offer UCI Health MyChart to his/her patients. IMPORTANT NOTE: Not all of our physicians are available to communicate using UCI Health MyChart, in which case communication will be through US mail service or telephone.

C. Activation Code

UCI Health MyChart is designed as a secure Internet-based environment through which you may receive confidential medical information about yourself. Once you sign up for UCI Health MyChart, you will be given a one-time-only UCI Health MyChart "Activation Code" that you will use to activate your account. You will then create your own UCI Health MyChart username, password and password hint. Please DO NOT SHARE your password with anyone. The individual UCI Health MyChart username and password you choose when you first log-on to UCI Health MyChart are the keys to protecting the confidentiality of your information. You are responsible for remembering your UCI Health MyChart username and password and for keeping them confidential.

  1. From the time you receive your temporary, one-time use Activation Code, you will have ONLY 90 days to log-on and activate your UCI Health MyChart account. After 90 days, if you have not activated your account, your temporary ten-digit "Activation Code" will expire. PLEASE activate your account as soon as possible so that you may begin using the services immediately, and not risk the inconvenience of having to sign up a second time.
  2. After you have activated your MyChart account, UCI Health MyChart will become the primary vehicle through which you will receive important messages from your healthcare provider, selected test results and other personal medical information. You will continue to receive communications via the United States Postal Service or other means from healthcare providers that have opted not to use UCI Health MyChart.
  3. Through UCI Health MyChart, you may request new appointments and prescription renewals 24 hours a day, 7 days a week. We ask that you first contact your pharmacy for existing, non-narcotic, prescription refills. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume and availability of staff. Therefore, for all urgent medical matters, contact your healthcare provider's office immediately, go to an emergency room, or call 911.
  4. You MUST provide us with your email address when activating your UCI Health MyChart account and keep your email address current via the "Personal Information" link under “Profile” in MyChart. No confidential information will be sent to your email, but you will receive email notification whenever new information has been released to your UCI Health MyChart account. You therefore MUST verify and update your email address via the "Personal Information" link under “Profile” to ensure that you receive notifications of newly released information in a timely manner.
  5. Once you have activated your account, your patient header display will include an active MyChart designation. You will then receive your personal health information as released by your healthcare provider in your personal UCI Health MyChart account.

D. Electronic Messaging

  1. Your healthcare provider may choose to select an associate to help manage and respond to your messages. These may include nursing or other administrative staff members who review messages sent via UCI Health MyChart and either respond or route the message to the healthcare provider. Therefore, use of UCI Health MyChart secure messaging may not be appropriate if the message includes sensitive information that you would only wish to discuss directly with your provider.
  2. All communications between you and your healthcare team using UCI Health MyChart are carried over a secure, encrypted connection directly into our electronic medical record. While you may receive Internet e-mail messages notifying you of new messages in your UCI Health MyChart Inbox, these e-mails will not contain any confidential medical information.
  3. Messages you send via UCI Health MyChart may become part of your permanent legal medical record. Once a message is made part of your permanent medical record, it will be accessible to current and future healthcare provider(s) and staff members who are appropriately involved with your care.

E. Medical Record Data

  1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including results of all tests, procedures and therapies.
  2. NOT ALL of your medical record may appear in your UCI Health MyChart. Posting of information is at the discretion of your individual healthcare provider(s) and UCI Health.
  3. Your healthcare provider(s) will maintain and update your electronic record at each office visit. However, if you believe any information displayed in UCI Health MyChart is not accurate, you are encouraged to notify your healthcare provider at your next office visit, submit a message to your clinic or contact UCI Health Medical Records.
  4. UCI Health reserves the right in the future to charge a fee for the use of UCI Health MyChart. You will be notified of this change and asked to re-apply if you agree to the fee and wish to continue with this service.

F. Information Does Not Constitute Medical Advice

UCI Health assumes no responsibility for how you use the information you obtain from UCI Health MyChart. You should consult directly with your healthcare provider or other care providers regarding your condition(s) and how UCI Health MyChart content may or may not apply. You may be accessing your own medical information via the UCI Health MyChart application and may wish to communicate with one of your healthcare providers about that or related information. Neither UCI Health MyChart nor any other application available through UCI Health MyChart is a substitute for appropriate and timely contact with your healthcare provider. UCI Health MyChart is not a substitute for consultation with your healthcare provider. You should never change or stop any course of treatment prescribed by your healthcare provider without first consulting him or her.

G. Termination of Access

UCI Health may restrict or terminate your access to UCI Health MyChart at any time for reasons including, but not limited to, scheduled system maintenance, if MyChart is used to send inappropriate or harassing messages or photos, or if an excessive number of visits are scheduled that are not medically necessary. If access is restricted for planned maintenance of UCI Health MyChart, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to UCI Health MyChart without advance notice. If your use of UCI Health MyChart violates these Terms and Conditions, we reserve the right to permanently limit your MyChart functionality or terminate your access to UCI Health MyChart.

H. Restrictions on Use and Ownership

All pages within UCI Health MyChart and any material made available for download are the property of UCI Health. The content on UCI Health is provided for our patients only. UCI Health’s name and its associated logo are the trademarks and service marks of UCI Health and University of California, Irvine. All other trademarks, service marks, and logos used in UCI Health MyChart, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on UCI Health MyChart should be construed as granting any license or right to use any trademark, service mark, or logo displayed on UCI Health MyChart without the express permission of UCI Health or the third party that may own the trademark, service mark, or logo.

UCI Health grants a limited license to each user to make personal use only of the Web site and the associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the Web site and the associated services, making any derivative of the Web site or the associate services, the collection and use of user email addresses or other users information, including, without limitation, health information or any data extraction or data mining whatsoever.

I. Accuracy and Integrity of Information

Although UCI Health attempts to ensure the integrity of UCI Health MyChart, it makes no guarantees whatsoever as to the correctness or accuracy of UCI Health MyChart. It is possible that the UCI Health MyChart could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform us so that it can be corrected. Information contained on the UCI Health MyChart may be changed or updated without notice.

J. Security

UCI Health MyChart uses encryption software. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using UCI Health MyChart, information will be transmitted over a medium that is beyond the control of UCI Health and contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USER OF THIS SERVICE. Once information is received by UCI Health MyChart, your medication and other medication information will be treated as confidential and given the same protection that all other medical records within UCI Health are given.

UCI Health makes no representations whatsoever about any other web site that you may access through UCI Health MyChart. When you access an external site, please be advised that it is independent from UCI Health and UCI Health MyChart, and that UCI Health has no control over the content on that web site.
